There is no physiologic reason for extraction sockets not to completely remodel after surgery, but chronic marrow ischemia will not allow proper healing, leaving residual lamina dura (X) for years thereafter. Apical regions often heal more thoroughly, leaving only the vertical dura remnants, which appear like streaks of lighting or falling rain on the radiograph. The X marks an area of greasy mush of osteonecrosis. |
